If you're reading this in the days or weeks after birth, sitting gingerly on a cushion with an ice pack close by, you're not alone, and what you're feeling is completely normal. Perineal tears and episiotomies are far more common than most women realise before they go through birth themselves, yet almost no one talks about the reality of recovering from one.

 

Why Tears and Episiotomies Happen

During vaginal birth, the perineum (the area of skin and muscle between the vagina and anus) stretches to accommodate your baby. Sometimes it stretches without tearing. Often it doesn't, and a natural tear occurs. An episiotomy is a surgical cut made by your midwife or doctor when they believe it will help your baby be born more safely or quickly than a tear would allow.

Tears are graded by depth:

  • First degree – skin only, usually healing within days to a couple of weeks
  • Second degree – skin and underlying muscle, the most common grade, typically healing within three to six weeks
  • Third and fourth degree – extending into or through the anal sphincter, requiring surgical repair and closer follow-up care, often over several months

Episiotomies are generally comparable to a second-degree tear in terms of healing time, though this varies depending on the individual.

 

What Slows Recovery

Understanding what works against healing is often more useful than a list of remedies. The main factors that tend to extend recovery are:

  • Infection – redness, increasing pain, discharge with an odour, or fever are signs to contact your midwife, GP or hospital promptly
  • Constipation and straining – this puts direct pressure on stitches and can delay healing or cause discomfort
  • Prolonged sitting or pressure – particularly in the first week
  • Poor nutrient status – tissue repair is a nutrient-demanding process, and depleted stores from pregnancy and birth can slow things down

 

What Genuinely Helps

Simple, evidence-based comfort measures make the biggest day-to-day difference:

  • Cooling the area with a perineal ice pack for the first 24–48 hours
  • Using a peri bottle with warm water when going to the toilet, rather than wiping
  • Sitting on a cushion or ring to take pressure off the area
  • Keeping the area clean and dry, and changing pads regularly
  • Gentle movement rather than complete bed rest, which supports circulation to the area

 

Supporting bowel comfort is one of the most underrated parts of recovery. Straining is genuinely one of the biggest aggravators of perineal pain, so adequate fluids, fibre, and gentle movement matter as much as anything applied topically.

 

Nutrition for tissue repair is where many women unknowingly fall short. Pregnancy and birth are demanding on the body's stores of iron, zinc, protein and vitamin C, all of which play a role in the body's natural tissue repair processes. Third and fourth-degree tears in particular warrant follow-up with a pelvic health physiotherapist, regardless of how recovery feels day to day.
